Prayers & Squares Quilt Request

Thank you for requesting a prayer quilt for your loved one! Please follow these steps to sponsor a quilt: 

  1. Request permission from the recipient and ask them for their prayer requests so worshippers know how to pray.
  2. Fill out the below form! You’ll hear from one of our members by phone or email to confirm the presentation date and make arrangements for quilt selection.
  3. If you have a photo of the recipient, please upload it with the form or deliver it to the church office by Wednesday before the quilt presentation.

Please take note of the required fields (those marked with *). All of this information is required for the form to go through! As confirmation, look for a message that states: "Thank you! Your submission has been received."
