Prayers & Squares Quilt Request

Thank you for requesting a prayer quilt for your loved one! These quilts are a free gift from our Prayers & Squares ministry. Please follow these steps to sponsor a quilt: 

  1. Request permission from the recipient and ask them for their prayer requests so worshippers know how to pray; as the sponsor, you can help them with the wording of their prayer requests.
  2. Fill out the below form! You’ll hear from one of our members by phone or email to confirm the presentation date and make arrangements for quilt selection.
  3. If you have a photo of the recipient, please upload it with the form or deliver it to the church office by Wednesday before the quilt presentation.
